LEADERS Q12 ASSESSMENT

The Q12 is a 12-question survey designed to measure workplace expectations, resources, recognition, and development. The goal of this assessment is for the staff to learn what we are doing well to support you as a leader, and ways that we can improve.

Keep in Mind

• The survey uses terminology such as “work”, “supervisor”, and “company” - please contextualize those to Collective, your Staff Member, and the Team you lead.

• If you serve on multiple teams, please fill out the assessment for each leadership role that you are in.

• If you get stuck, or need clarity, please message your staff member.

• Be honest and thoughtful.
